We value your time and that is why it is very important that you complete the forms as accurately and thoroughly as you can. Legible, accurate, and thorough completion of the intake paperwork will help us better treat you and make your visit as efficient as possible. If you are unable to fill out the paper work prior to your appointment, we ask that you arrive 20 minutes early to your appointment to fill out the intake paperwork in our office.
